When the shuttle Columbia exploded it was just as devastating as the shuttle Challenger that exploded in the late 80’s. These types of tragedies are not supposed to happen ever. It is up to all crews responsible such as the mission control team, to make sure that every piece of the shuttle is up to par before going into space.
I would suggest to the members of the mission control team, that if they have doubts, it is very important that you speak up and let someone know that you think something may be wrong. Although you may not have all the evidence to support your decision, you have to make your superiors believe you and hope that they take a second look. This has to happen because 1. It’s just common sense and 2, what harm is it to check over the shuttle again to avoid another tragedy as this was. I would also recommend that maybe they take it upon themselves to go over things on their own. Someone needs to take the initiative and step up to the plate. If this would have happen, then maybe we would be talking about something else right now instead. I just find it hard to believe that someone would not speak up about this situation and put something into place to make sure something like this will never happen again.
After all the tragedies that NASA has been through, they finally settled down and came up with plans and scenarios to prevent this from happening again in the future. They are always double checking themselves and also not second guessing themselves as well. They are coming together as a group and listening to each other more and holding themselves accountable for their actions. They are also avoiding groupthink and this is a plus, because this means that they trust each other to get the job done (Johnson, 2012). The crew actually flipped the shuttle Endeavour over as it orbited the space station, allowing the station crew to identify the location of the size of the gouge. All in all the NASA team came together at the right time...
